Facility Evaluation Report
Licensing Program Analyst (LPAs) Coppo arrived announced to conduct a Required - Year Inspection and met with Ralph Gause, Licensee.
LPA and licensee toured the facility. There are currently no residents in care. Licensee does still plan to open the facility as an ARF, but as of yet has not pursued getting vendorized with North Bay Regional Center. Facility has been rented out as of April of 2025. Licensee will immediately notify CCL once facility has clients in care. A visit will then be conducted at that time.
Potential resident rooms are currently furnished with renter's items but when/if clients are in care the rooms will be equipped per regulation. Water temperature measured 121.2 degrees F which is not within regulation. However, per licensee, before clients are in care the water temperature will be adjusted down. All medication and client/personnel records are to be stored in the locked hall closet. Cleaners, toxins, and items that can pose a danger will be stored in a locked cabinet in the garage. LPA discussed some documents required upon admission of clients and hiring of personnel. Licensee does not plan to handle client's cash resources.
The fire extinguisher located in the laundry room was last inspected 12/19/24 and found to be charged. Facility continues to have fire inspections. All exits are accessible and free of debris. North side of house has various items stored partially obstructing the fire exit. However, licensee will be removing the items very soon as tenants do not wish for items to be stored there. Administrator Certificates are current; Liyan Gause, 7035952735, expires on 3/16/27.
