Facility Evaluation Report
Licensing Program Analyst (LPA), Alviso, conducted a Required- 1 Year visit, on 4/29/25 at approximately 2:00pm, and met with Administrator John Wotring and Cynthia Tilker, Program Director. There are currently twelve (12) clients participating in day program services, with nine (9) of them attending today.
Facility has a required infection control plan. Facility has a required emergency disaster plan. Fire clearance approval is for twenty-five (25) non-ambulatory.
The facility is sprinkled, and has a hard wired smoke alarm system. The facility has four(4) carbon monoxide detectors.
LPA toured the day program with the Administrator and Program Director. Facility provides lunch to all clients of the day program. Facility kitchen, activity areas, resident bathroom, conference area, staff offices, and staff bathroom, were observed to be clean and orderly.
The exits were all cleared and unobstructed. The common areas, bathrooms, hallways all had sufficient lighting for all clients use.
LPA observed staff interacting with clients' in attendance, and LPA observed clients participating in activities being provided by staff. Facility's fire extinguishers were serviced and tagged as required. Cleaners/disinfectants were locked up and inaccessible to clients' in care. LPA observed numerous activity games, equipment, and supplies for the day program attendees.
