Facility Evaluation Report
Licensing Program Analyst, (LPA), conducted a case management, on 2/26/26 at approximately 1:30pm, and met with Abraham Naizgy, Administrator, and Azieb Tecle, Caregiver.
Facility has four (4) clients in care. Three of the four clients attend day program at this time.
LPA reviewed four (4) client records, and obtained copies. It was identified that the facility had a second admission agreement that they had completed with clients in care and responsible parties. The admitted clients are regional center placed, and the home is vendorized with the North Bay Regional Center (NBRC). Clients placed by NBRC have the regional center admission agreements that are signed by required parties. This admission agreement is the valid contract of the placement and cost to admit the individual client into care at Naizghis facility.
The second admission agreement completed by the Naizghis Licensee/Administrator is not valid, and is to be removed from the client files. Abraham Naizgy, Licensee/Administrator stated their understanding of the above, and agreed they will remove the second admission agreements. The admission agreements had both adult residential regulations and residential care for the elderly regulations combined in the agreements. Naizghis is an adult residential facility.
LPA discussed the regulations regarding 80068- admission agreements, 80072-personal rights, and 80061-reporting requirements, with Licensee/Administrator, Abraham Naizgy, who stated their understanding of the above.
LPA gave three (3) technical violations, LIC9102 forms, regarding 80068- admission agreements, 80072-personal rights, and 80061-reporting requirements.
Technical violations are from the California Code of Regulations (CCRs), and/or the Health and Safety Code.
