Facility Evaluation Report

At approximately 9:30 AM Licensing Program Manager (LPM) Victoria Bertozzi and Licensing Program Analyst (LPA) Robert Frank arrived at the home for the purpose of voluntary closure of this licensed Residential Care Facility for the Elderly and met with Licensee Tiffany Dizon.

The licensee initiated this facility closure with plans of closing due to plans of no longer wanting to operate as a licensed facility. LPA inspected all rooms and the exterior of the building and found no evidence that would suggest that clients are residing on the premises.

Closure of this facility has been finalized. Facility was closed effective 08/7/2026. Licensee turned over copy of License during today's inspection.
