The inspector’s account

Licensing Program Analysts (LPAs) Victoria Bertozzi and Helena Rummonds arrived unannounced to conduct a Case Management inspection and met with staff, Myla Rublico

LPA is following up regarding a recent incident where a resident was able to obtain a knife and physically threatened staff with it. Per interview, the knife had gone missing and staff were unable to find it for multiple weeks. LPA confirmed that facility has a locked cabinet for knives and staff are unsure how resident obtained the knife. Per interview, no clients were threatened and were not aware of the incident while it was happening.

Deficiencies cited from the California Code of Regulations, Title 22, Division 6 of California Regulation. Appeal rights given. Failure to correct the deficiency and/or repeat deficiencies within a 12 month period may result in civil penalties.
