Facility Evaluation Report

Licensing Program Analyst (LPA) Cuadra conducted an unannounced case management visit and met with Jacqueline Galasso, LVN Nurse; Administrator, Mary Clein Galang arrived later. The purpose of the case management visit was to follow up on a deficiency cited during an annual visit conducted on 12/11/25.

On 12/11/25, it was discussed about the importance to keep personnel records including staff training records binder available to the licensing agency to inspect, audit during normal business hours. At that time, Administrator disclosed that a participant accidentally shredded them and administrator was in the process of re-train staff to have records available for licensing agency to review, but binder was not available during LPA's visit. During today's visit, LPA reviewed staff training binder and confirmed that staff have been receiving training as stated per regulation.

No deficiencies cited during today's inspection. Exit interview conducted with Administrator and a copy of this report was given.
