The inspector’s account

Licensing Program Analyst (LPA) Marisol Cuadra conducted an unannounced case management visit and met with Jacqueline Galasso, LVN Nurse; Administrator, Mary Clein Galang arrived later. The purpose of the case management visit was to follow up on a self-incident report dated 8/3/2026.

Per incident report, Upon arrival to day program, participant (P1) was observed with a large area of bruise to their right forearm. Facility nurse staff assessed the bruising, skin remained intact with no open areas or drainage noted, P1 did not showed any signs of discomfort or distress during assessment. An ice pack was applied to the affected area for comfort and their responsible party was contacted. According to home staff, P1 was agitated during the evening of 8/2/2026, repeatedly struck their right forearm against the armrest of their commode.

During today's visit, LPA learned that the day program nurse continued to monitor the affected area to ensure timely medical attention if needed while continue to utilize established behavior support, de escalation strategies to minimize episodes of agitation to reduce the risk of self injurious behaviors while P1 is at the program.

No deficiencies cited during today's visit. Exit interview conducted with facility nurse and copy of this report was given.
