The inspector’s account

Licensing Program Analyst (LPA) Cuadra conducted an unannounced Annual Required – 1 yr. and met with Licensee Emma Mutunga. Annual fees are current. Required postings observed.

LPA/Licensee toured the facility around 9:00am. The facility was found to be clean and at a comfortable temperature with all exits free from obstruction. Resident’s bedrooms furnished per regulation. Fire Extinguishers were charged on 12/4/24, smoke detectors & carbon monoxide detectors were found to be operational during the visit. Last disaster drill was conducted on 9/16/25. Hot water temperature measured 110.3 and 110.1 degrees F which is within acceptable regulations of 105 to 120 degrees F. There was a supply of hygiene products and paper products. Toxins are stored in a cabinet in the garage that was locked. Medication is centrally stored and locked in a closet located in the kitchen. The facility has one week of perishable food and two days of nonperishable food observed stored and handled as stated per regulation.

At approximate 9:30am LPA initiated a review of three staff and four resident files. All four residents care plans and medial assessments were updated. All staff files have active CPR/first aid certificates and required training hours. Administrator Certificate for Emma Mutunga, 7000702740, expires 4/18/2027. Medication and medication records were reviewed.

Licensee agreed to provide updates of the following by 11/10/25: LIC500 - Personnel Report and LIC308 Designated Administrative Responsibility.

No deficiencies found during today's visit. Exit interview was conducted with Licensee and a copy of this report was provided.
