Facility Evaluation Report
Licensing Program Analyst (LPA) Cuadra arrived unannounced to conduct a plan of correction visit to continue staff training records review which are part of annual required inspection which was held on 4/1/26 and met with Licensee, Tony Guevarra. On 4/1/26, LPA Cuadra completed a physical inspection of the facility, and reviewed residents and staff records except staff training records were not available for review at the facility during that visit.
During today's visit, LPA reviewed training records for three staff (S1, S2 & S3) which were completed per regulation. Also, LPA cleared the deficiency and observed areas of concerns that needed repair were completed.
No deficiencies were found during today's inspection. Exit interview conducted with Licensee and copy of this report was given.
