Facility Evaluation Report

Licensing Program Analyst (LPA) Cuadra conducted a case management visit to cite deficiencies discovered during a complaint investigation and met with Licensee, Tony Guevarra. LPA conducted risk assessment call with Licensee, Tony Guevarra prior to the visit.

LPA learned through interviews on 7/29/21 with Licensee and Administrator had failed to submit Eviction Notices issued to Client (C1) to CCL within 5 days of occurrence as stated per regulation (85068.5 Eviction Procedures)

Deficiencies cited from the California Code of Regulations, Title 22, Division 6 of California Regulation. Appeal rights given. Failure to correct the defi ciency and/or repeat deficiencies within a 12-month period may result in civil penalties.
