Facility Evaluation Report
Licensing Program Analyst (LPA) Cuadra arrived unannounced for the purpose of conducting a case management visit to check in areas of concerns of food services availability for clients in care previously addressed with the Licensee, Tony Guevarra.
During today's visit, at approximately 11:53am LPA/staff observed three apples as the only snacks available on the living room table for a census of sixteen clients in care. Staff went to the kitchen area and grabbed ten bananas that appeared to have dark skin like rotten not fresh appearance as snacks for clients in care and placed them in a bowl located in the living room accessible to clients in care. The kitchen remains locked when staff are not in the kitchen . According to staff, staff puts them out in the morning, then they fill it up again in the afternoon. Licensee states that they can't afford to keep placing snacks out there for them, because the facility doesn't makes enough money to buy snacks every day.
Deficiencies cited from the California Code of Regulations, Title 22, Division 6 of California Regulation. Appeal rights given. Failure to correct the deficiency and/or repeat deficiencies within a 12 month period may result in civil penalties.
Exit interview conducted with Licensee and copy of this report was given.
