Facility Evaluation Report
Licensing Program Analyst (LPA) Cuadra conducted a case management inspection-other and met with Licensee Tony Guevarra.
On 11/8/25 Licensee left a voicemail notifying the Department about client (C1) who voluntary AWOL'd the facility with all their belongings. On 11/10/25, there was a follow up incident report stating that on 11/8/25 at approximate 10:15am C1 yelled at staff using aggressive behavior, so staff called 911, but C1 have packed all their belongings and voluntary left the facility prior to police arrival. The facility notified C1's responsible party.
During today's visit, LPA reviewed pertinent documentation. According to C1's physician report dated 9/16/24, C1 is able to care for themselves and capable of leave the facility unassisted. Also, C1's care plans confirms impact of their behavioral diagnosis. Based on records review, the facility staff has followed their own polices and procedures regarding AWOL/Elopement.
No deficiencies cited during today's inspection.
Exit interview conducted with the Licensee and copy of this report was given.
