Licensing Program Analyst (LPA) Erik Gonzalez Campos arrived unannounced on 06/02/2022 and met with Executive Director, Safoora Ahmed.

LPA conducted a visit to return file for resident (R1). Executive director reviewed contents of file and and confirmed file was returned complete and undamaged per regulation.

Exit interview conducted with administrator and a copy of this report emailed to the facility.
