Licensing Program Analyst (LPA) Cuadra arrived unannounced for the purpose of conducting a case management to follow up on two self-incident reports submitted to the department and met with Cassie Reed, House Program Manager. During today's visit, LPA met with new program manager and obtained updated contact information for the facility.

First incident report described that on June 20, 2025 at approximately 8:26pm, staff heard client (C1) and client (C2) scuffing chairs in the dining room. Staff immediately went to the dining room, observed both clients to be agitated and both reported they pushed each other. After interviewing both of them confirmed to staff that they did push each other, staff proceed with their internal protocol to call the police, who came evaluated the situation and determined that they both could remain at the facility because there was no harm assessed, then both clients apologized to each other. Their responsible party was notified and no further incidents with other clients have occurred since this incident. However, C1 has been exhibiting inappropriate behavior on 7/22/25 and 7/24/25 including but not limited to swearing, speaking inappropriately towards staff, slamming doors with possibility of causing property damage, resulting in a Wellness agreement was issued and signed on 7/30/25. LPA had a conversation with new program manager regarding the importance of reporting requirements submitted to the department when there are ongoing behavior issues with clients in care.
