Facility Evaluation Report
At approximately 2:50 PM, Licensing Program Analyst (LPA) Robert Frank arrived unannounced to conduct a Case Management inspection and met with Director, Sarah Magnani. The facility submitted one (1) Incident Report (IR) for Client 1 (C1).
The IR stated that on Wednesday, 2/25/2026 at approximately 4:15 PM, client C1 showed facility Director Sarah Magnani a dime size faint yellow bruise on their upper right arm. Client C1 alleged that a staff member (staff member S1) had punched them in the arm at approximately 4:15 AM earlier that morning. Client C1 stated that staff member S1 was putting dishes away very loudly. Client C1 then asked staff member S1 why they were being so loud, After asking that question, client C1 alleged that staff member S1 punched them. There were no witnesses to the incident. Client C1 declined medical attention and First Aid was not required for the bruise. Staff member S1 was placed on administrative leave pending an internal investigation. In addition to filing the IR with Community Care Licensing, the facility notified Adult Protective Services (APS), Local Law Enforcement, the local Ombudsmen, the California Department of State Hospitals and the California Department of Developmental Services. Local law enforcement declined to investigate the incident. Additionally, the facility filed an SOC-341 Report of Suspected Dependant Adult/Elder Abuse with said entities. The Progress Foundation's internal investigation determined that there was not enough evidence to corroborate Client C1's allegation. Staff member S1 was taken off of administrative leave.
No deficiencies cited during today's visit.
Exit interview conducted. Copy of report and LIC-811 Confidential Names discussed and provided to Director Magnani. Signature on form confirms receipt of documents.
