Facility Evaluation Report

Licensing Program Analyst Willis arrived unannounced to conduct a Case Management inspection and met with Administrator, Audrey Land.

LPA is following up regarding a self reported incident where client, C1 had two insulin medications that were mixed up resulting in C1 taking an incorrect dosage of one of the medications. Client was sent to the hospital and was medically cleared after about four hours because they did not have any adverse affects.

Facility has conducted additional medication training for involved staff.

Deficiencies cited from the California Code of Regulations, Title 22, Division 6 of California Regulation. Appeal rights given. Failure to correct the deficiency and/or repeat deficiencies within a 12 month period may result in civil penalties.
