The inspector’s account

At approximately 8:35 AM License Program Analyst (LPA) Robert Frank arrived unannounced to conduct a Case Management visit and met with Program Director (PD) Laurel Pope. LPA was informed that there are currently eight (8) residents in care.

On 11/7/2025 an LIC 624 Unusual Incident/Injury Report was submitted to Community Care Licensing by the facility. Today's visit was related to the submitted LIC 624. During today's visit LPA conducted an interview, requested and reviewed documents.

No deficiencies cited during today's visit.

Exit interview conducted. Copy of report discussed and provided to PD Pope. Signature on form confirms receipt of documents.
