Facility Evaluation Report
Licensing Program Analyst (LPA) Christi Coppo arrived unannounced to conduct a Case Management visit and was greeted by licensee Domonique Thomas.
On 4/9/26 CCL received an Incident Report indicating resident (R1) left the facility at the early morning hour. Per facility report, R1 was showering around 12:30am and had informed staff they were leaving the facility. Staff reminded R1 of the House Rules which states the residents must observe a curfew between 11:00pm and 6:00am. R1 indicated their disapproval of the curfew. Staff reminded R1 that they agreed to the curfew upon admission. At 12:51am R1 left the facility out the front door and returned to the facility at approximately 2:00am. R1 banged on the front door to be let in the facility. Once they were let in the facility R1 went directly to their room.
During visit, LPA reviewed physician’s report for R1. Physician’s report states that R1 can leave the facility unassisted . LPA discussed with licensee that when a physician’s report states a resident can leave a facility unassisted and they leave without supervision, it is not considered an elopement. After incident, facility held meeting with R1's care team, including a behaviorist that facility has hired to help offer support to facility residents in achieving their goals and help with behaviors. A meeting was held to discuss support options for R1 but the meeting was canceled by the Regional Center case worker. Facility was able to still meet with behaviorist and a behavior contract was made. However, per licensee and staff, R1 has yet to be successful in satisfying the criteria outlined in the contract. Staff continue to work with R1 to address dangerous behaviors. R1 remains resistant and has chosen to look for alternative housing. As of today, R1 is still residing at the facility but is actively seeking placement out on their own. Staff continue to try and support R1 and the behavior contract goals. LPA reviewed behavior contract and behavior frequency logs dated December 2025 through current.
Exit interview conducted with licensee and a copy of this report given.
