Licensing Program Analyst (LPA) Alviso conducted a case management- incident inspection, on 3/11/25 at approximately 3:58pm, and met with Administrator Megan Leone.
LPA is conducting a case management to obtain more information on a resident incident that was reported by the Administrator. LPA reviewed resident (R1's) records. LPA requested copies of specific resident records. LPA also requested a current staff roster. Copies were provided to the LPA.
Exit interview was conducted with Administrator Megan Leone.
