Facility Evaluation Report
Licensing Program Analyst (LPA) Cuadra arrived at facility unannounced to conduct a case management to follow up on exception request submitted to CCL on 11/20/25, which it needs additional documentation. Today, LPA was greeted at the door by staff Melissa Esteban. Administrator Hanna Peralta was available by phone and provided information about the status of the documentation needed for exception request.
During today's visit, LPA was given additional letter needed as supporting documentation to approve the exception request for client (C1) for postural support.
No deficiencies cited during today's visit.
Exit interview conducted with staff and copy of this report was given.
