Facility Evaluation Report

Licensing Program Analyst (LPA) Alviso arrived to conduct a case management inspection, 9/16/25 at about 4:00pm, and met with Cathie Clark, Licensee/Administrator.

The Licensee stated that they want to surrender the license, as they no longer able to operate the care facility; Licensee had all residents transferred out of the facility by their responsible parties. All of the residents/responsible parties were notified that the Licensee had to move due to the owner of the home wanting to use the home for family.

LPA observed that the home was empty of furnishings through the front glass door; There were no persons observed on the front grounds, and no one answered the front door while the LPA was at the home.

Licensee arrived to the home to meet with the LPA out front of the former facility.

The license is no longer valid. If wanting to operate a care home in the future, an application would need to be filed with the Department, and applicant would need to go through the application process.

The facility license is closed today, effective 9/16/2025.
