Facility Evaluation Report

Licensing Program Analyst (LPA) Cuadra arrived unannounced to conduct an inspection regarding an incident filed on 11/18/2025 and met with staff, Sheena Helems. Licensee, Tess Hayes was not able to come to the facility, but she was available by phone and gave authorization to staff to sign the report.

LPA is following up on a self-reported incident where client (C1) had a fall while in their room on 11/16/25, but was able to get up independently. However, on 11/17/25 at about 7:10am, C1 had another fall and required staff assistance to stand; Following the second fall, C1 was unable to keep down food or medication and began vomiting and staff contacted the ambulance immediately to transport them to the Memorial Hospital.

During today's visit, LPA learned that the Licensee have visited C1 at the hospital, but the hospital social worker is gathering additional information regarding financial inquiries prior to provide a diagnosis to the Licensee. Per Licensee, C1 is not conserved and their responsible party handles their finances, so their responsible party is in the process to provide requested information to the hospital. As of today, C1 is still hospitalized and receiving treatment. Previously, C1 had a history of ongoing kidney issues, which it could be the reason of their illness. Licensee agreed to provide updated information to the Department as soon as they have additional information.

No deficiencies cited during today's visit. Exit interview conducted with staff and copy of this report was given.
