Facility Evaluation Report
Licensing Program Analyst (LPA) Marisol Cuadra arrived unannounced to conduct an Annual Required Inspection and met with Licensee, Russell Loewen. Fees are current. Clients were attending day program.
LPA/staff toured the facility and observed the following: Facility was a comfortable temperature and passageways were free from obstruction. Client rooms were furnished per regulation. Water temperature in client bathroom read at 116.2 which is within regulation of 105 and 120 degrees F. At least two days of perishable and one week of non-perishable foods were available. Toxins were stored in a locked cabinet in the laundry room and a locked cabinet under the kitchen sink. Fire extinguishers were last inspected August, 2025. Facility has a fire pull system. Smoke detectors and one carbon monoxide detector were tested and operational. Most recent Fire/Disaster drill was conducted 4/30/26.
LPA initiated file review at 2:20 pm. Two client and two staff files were reviewed. First aid certificates for staff are current and training hours are complete. Administrator Certificate for Vickie Loewen, 7034428735 expires 12/8/2026. Medication and medication records were reviewed. Cash resources were reviewed. Medications are centrally stored and locked. During file review, LPA learned that now the facility has two clients in care, where one of them is older than 60 years old which results in 50 percent of the census at the facility. LPA had a conversation with Licensee to inform them about the need to request an exception to the department in order to retain client. Licensee agreed to submit documentation listed below along with an exception request letter: The client’s responsible person/agency letter support client’s placement in this facility, all staff required training hours including dementia training hours, client’s needs and services plan, and the medical assessment (LIC 602). Licensee already submitted updated copies of the following: LIC500 (Personnel Report) & surety bond.
No deficiencies cited during today's inspection. Exit interview conducted with Licensee and a copy of this report was given.
