Facility Evaluation Report

At approximately 2:40pm Licensing Program Analyst (LPA) Christi Coppo arrived at California Mentor-Calistoga House for the purpose of a closure inspection to confirm that there are no residents present at the facility pursuant to their notification of closure on 10/28/25.

On 11/3/25 LPA confirmed with North Bay Regional Center's Quality Assurance Supervisor that the last remaining resident was moved from the facility and is settling into their new home as of 11/1/25.

LPA was greeted by Chelsea Gonzalez, California Mentor Regional Director (CMRD).LPA inspected the interior and exterior of the facility and confirmed that no residents are residing at the premises. Facility was being cleaned out and all furniture was actively being removed upon LPA's arrival. LPA confirmed with CMRD that premises was rented for facility use.

The facility license was surrendered. The closure of this facility has been finalized. Facility is closed effective 11/6/2025.
