Licensing Program Analyst Bertozzi arrived unannounced to conduct a Case Management inspection and met with Administrator, Maria Cortes.
LPA is following up regarding a self-reported incident where a resident fell and was observed to have a bump on their head so was sent to the hospital. Facility was later notified of resident's passing while in the hospital and was not given a cause of death. Per conversation with Administrator and review of file, resident did not require 1:1 supervision.
No deficiencies cited during this inspection.
