Facility Evaluation Report
Licensing Program Analyst (LPA) Christi Coppo arrived unannounced to conduct a POC visit and was greeted by designee. Administrator Edward Alicdan Jr. was also present at facility.
On 4/23/26 LPA conducted a NCC visit at facility and recited deficiencies for which the plans of correction were not completed. Deficiency of regulation 87465(h)(5) was recited with a plan of correction due date of 4/24/26. Deficiency of regulation 87303(a) was recited with a plan of correction due date of 4/24/26. As of today, 5/5/26, the plans of correction for these deficiencies have not been received. Therefore, LPA is issuing civil penalties for failure to correct.
Civil penalties in the amount of $100 per day for nine (9) days, from 4/27/26 through 5/5/26, are being assessed for failure to correct deficiency of regulations 87465(h)(5) and 87303(a), for a total of $900 each. The penalty of $100 per day will continue to accrue until the plan of correction is received for each deficiency.

Deficiencies cited from the California Code of Regulations, Title 22, Division 6 of California Regulation and the Health and Safety Code. Appeal rights given and discussed with designee.
