The inspector’s account

Licensing Program Analyst (LPA), Alviso, is conducting a Required - 1 Year visit, on 5/6/25 at approximately 1:45pm, and met with Ana Nunez Rueda. LPA observed three other caregivers on duty;

DSP Troy, and Meagan & Rylee, DSPs in training.

The House Manager, Lexus Fletcher (Snyder), arrived to meet with the LPA. Administrator Catherine Brenegan arrived later during the inspection to meet with the LPA.

Facility has an approved fire clearance for four (4) , and two (2) non-ambulatory clients; Room #4 is cleared for non-ambulatory use. There are currently five (5) residents in care.

The facility does have a required infection control plan. Facility does have a required emergency disaster plan. Last emergency disaster drill was held on 4/9/25 and 1/21/25.

The LPA reviewed five (5) resident files.

The LPA reviewed five (5) staff files. LPA reviewed staff training. All staff have criminal record clearance as required. All staff have first aid certification and CPR certification.

LPA toured the facility with House Manager Lexus Fletcher (Snyder). Hot water was measured at 117.4 degrees Fahrenheit, which is within regulation.
