Facility Evaluation Report
Licensing Program Analyst (LPA) Cuadra conducted an unannounced case management and met with James Cox (Executive Director). The purpose of the case management visit was to obtain additional information regarding a recent incident and SOC 341 that was self-reported by the facility on 6/17/2022.
LPA received unusual incident report and SOC341 involving participant (P1) alleged physical abuse dated 6/17/2022. Per incident report, On 6/17/22 P1 was picked up by facility bus driver and it was noticed a red bruising-looking area around their right eye. P1 reported to the driver that this was caused by P1's father. The bus driver notified the Director of Education and Supervisor privately conduct an assessment and inquired with P1 about what happened, P1 stated that their eye doesn't hurt and confirmed that injury was caused by their father due to participant had been acting up the previous night because didn't want to change their dirty socks, and their father had thrown P1 to the floor and pointed to an area on the right side of their upper lip, and said their lip had been injured in the altercation too. P1 stated that the police had been called and they came to their home. The Supervisor notified APS and P1's service coordinator who was not aware of the incident. Supervisor also cross reported to Santa Rosa Police Department who confirmed that they had received a call at 7:25pm the previous evening were the M-Response (Mental Health) Team as well as other officers responded to the call. It was reported that P1 could be heard screaming inside the home and that three people were home at the time (P1, P1's father and a person that the police identified as P1's "mother"). Police officer noted that P1's mother had been thrown to the ground at some point during the altercation. There was no police report written, but an event number is SR-221670298 was provided.
During today's visit, LPA was informed that this is the first time that an incident like this happened to P1 who lives with their father and step-mother, P1 is verbal, ambulatory and not conserved. LPA was unable to talk to participant because today they did not attend to Day Program until next Thursday and Friday. No further incidents have been reported.
No deficiencies cited during todays inspection. A copy of this report was provided to Executive Director.
