Licensing Program Analyst (LPA) Cuadra arrived unannounced to conduct a case management visit and met with Stephanie Rodriguez, Service Director. The purpose of this case management-incident visit is to deliver substantiated findings of self-report incident to Community Care Licensing.

On 7/2/2025, the Department received a self-report incident indicating possible sexual assault of client (C1) in local park. Per the incident report, on 7/1/25 a community person contacted local police reporting inappropriate behavior in park gazebo. C1 had an arranged meeting with alleged abuser (I1). The incident was investigated by the department as an alleged lack of care and supervision resulting in a resident being sexually abused. Based on a police report obtained on 7/25/2025 alleged abuser was detained on 7/2/2025. C1 was assessed by hospital staff and taken for further examination. During the course of this investigation, The Department investigator (IB) conducted interviews with staff, residents and other witnesses, and reviewed records associated to the involved client (C1). Based on records review, on 7/1/2025, Windsor Police Department was notified that C1 was sexually assaulted in a public park by I1 a person not associated to the facility. C1 was at a public park with their day program group included staff (S1), and other clients. During the outing, S1 allowed C1 to leave their group to say “hi” to I1, who was supposedly be C1’s friend, but C1 was out of S1’s line of sight for about five to ten minutes when I1 sexually abused C1. IB conducted interviews with the Service Director who believed that S1 allowed C1 to go too far from the group and that they wouldn’t have trained any staff to allow their clients to leave their group as far as C1 did.
