Facility Evaluation Report

An office meeting was conducted today in the Santa Rosa Regional Office. Present at the meeting were Licensing Program Manager Bethany Moellers, Licensing Program Analyst Marisol Cuadra, Paula Finley, Chief of Services , Luana Vaetoe Administrator and Stephanie Rodriguez Service Director.

The purpose of the office meeting was to discuss concerns that have been identified by the Licensing Agency regarding the operation of this facility including but not limited to: Incident Report – Priority 2 investigation conducted by the Department that has been substantiated in a case management visit for Personnel Requirements violation due to the facility failed to provide safety and adequate supervision to a client resulting in a report of client being sexually abused. An immediate civil penalty in the amount of $500 was issued during the case management visit.

According to Administrators, the following preventive measures have been implemented to ensure safety and adequate supervision of clients in care: rotating staff schedules, staff meeting and in-service training was conducted with all staff.
