Facility Evaluation Report

Licensing Program Analyst (LPA) Cuadra conducted an unannounced case management visit and met with Stephanie Rodriguez, Service Director. The purpose of this case management visit is to follow up on a self-report incident report submitted to Community Care Licensing (CCL) dated 7/2/25 involving a participant observed in public possible engaging in inappropriate sexual activity. The Department received police report on 7/24/2025 with additional information.

During today's visit, LPA requested pertinent documentation including participant's physician report, individual program plan dated 2/24/23 and interpersonal behavior plan (ISP) dated 6/11/25.

The Department will review information obtained to determine if further actions are needed.

Exit interview conducted with Service Director and a copy of this report was given.
