The inspector’s account

The California Department of Social Services (CDSS) Community Care Licensing (CCL) Santa Rosa Regional Office conducted an in office, Non-Compliance Conference today, 7/8/26, with UCPNB Sonoma Day Program-496804147, licensee- United Cerebral Palsy of the North Bay, INC. This meeting today will also address concerns with operations in Licensee’s three (3) other licensed day programs, see LIC809s on each facility.

Licensing staff in attendance: Regional Manager Carla Nuti-Martinez, Licensing Program Managers, Bethany Moellers, and Victoria Bertozi, and Licensing Program Analysts, Dina Alviso, Elias Magdaleno, and Ali Deniz.

NBRC staff attendees: Tonya Fagan, and Caitlin Igoe, by Microsoft Teams and/or via phone

UCPNB attendees: Administrator/Area Director Jerry Corradi, and Christina Isetta, the interim CEO.

The purpose of this office meeting was to discuss areas of concern in the facility operations regarding client incidents, and recent and/or past inspections.

UCPNB Sonoma Day Program 6/23/2026 – Under investigation: Staff did not follow appropriate staff to client ratios, Staff did not meet training requirements and Staff did not properly report incidents

6/6/2025- RO received a self-reported incident report (SIR), updated SIR submitted on 6/11/2025 alleging sexual abuse. SIR informed of inappropriate behavior between staff (S2) and clients (C1). **S1 is a NBRC client and RO processed NIE case.
