Facility Evaluation Report

Licensing Program Analyst (LPA) Alviso arrived unannounced to conduct a continued annual inspection, on 9/11/2025 at approximately 3:00pm, and met with Lead caregiver, Nancy Brasher.

LPA provided to Lead staff, Nancy Brasher, the updated facility license, reflecting correct fire clearance approval of "6 ambulatory" residents only.

Currently there are four (4) clients in care.

The facility has a fire clearance for six (6) ambulatory clients. Facility is not fire cleared approved for non-ambulatory or bedridden residents.

LPA reviewed four (4) resident files. All files were complete.

LPA reviewed three (3) staff files. All staff have required criminal record clearance. All staff have required first aid certification. LPA reviewed staff training.
