Facility Evaluation Report
Licensing Program Analyst (LPA) Alviso arrived to conduct a Required -1 Year inspection, on 8/20/2025 at approximately 3:55pm, and met with caregiver, Nancy Brasher. LPA observed another caregiver on duty during the inspection.
Currently there are four (4) clients in care.
The facility has a fire clearance for six (6) ambulatory clients. Facility is not fire cleared approved for non-ambulatory or bedridden residents.
Facility was observed to be clean and orderly. The food supply was observed to be sufficient with perishable and non-perishable items. All medications were locked up and inaccessible to clients in care. Sufficient lighting was observed in the facility common areas, bathroom, and resident rooms.
Disinfectants/cleaners were observed to be locked up and inaccessible to clients in care. Fire extinguishers, two (2), were serviced and tagged as required. Facility has a working carbon monoxide detector, as required. The facility has required working smoke alarms. Hygiene products, paper products, and linens were observed to be a sufficient supply for clients in care. Bathroom was observed to have been updated, it was clean, had toilet paper , and paper towels. Hot water was measured at 115. degrees Fahrenheit.
The LPA will continue the annual inspection at a later date.
