Facility Evaluation Report
Licensing Program Analyst (LPA), Alviso, conducted a Required -1 Year visit, on 6/29/26 at approximately 9:35am, and met with Administrator/Program Manager, Jesse Carbone, and Assistant Manager, Clara Avina.
Day program has a fire clearance for sixty(60) capacity, which includes seven(7) non-ambulatory. During today's inspection the facility was operating with twenty (20) clients who attended services today. The facility has a required infection control plan. The facility has a required emergency disaster plan; Emergency disaster drills are being conducted.
The LPA reviewed six (6) client files. All the client files were found to be complete.
The LPA reviewed six (6)staff files. All staff have First Aid, AED, and CPR certification. All staff have DOJ criminal record clearance as required. LPA reviewed staff annual training. LPA provided to PIN 25-11, Adult Care Facilities Reference Guides To Administrator, Staff, And Volunteer Training Requirements, for Administrator's review, to help ensure all staff training is completed as required. Administrator stated their understanding of he provided PIN.
Hot water was measured at 115. degrees Fahrenheit. Facility has a working carbon monoxide detector. Facility has five (5) fire extinguishers that are serviced and tagged. All exits were free and clear of obstruction. Facility's large mounted first aid kit had a large supply of all required items, including required first aid guide. Cintas comes out monthly to check the first aid supplies and restock items as needed. Facility has an AED machine mounted in it's storage case, it is located by the first aid case.
