Facility Evaluation Report
Licensing Program Analyst (LPA), Alviso is conducting an Required-1 Year inspection, on 06/22/26 at approximately 3:35pm, and met with Administrator Leonardo Dela Cruz. LPA observed two caregivers, Rolando "Klowie" Correa, and Josephine Dobel on duty.
Currently there are two (2) clients in care. Facility has an approved fire clearance four (4) non-ambulatory clients. All client rooms are private. The facility does have a required infection control plan, and a required emergency disaster plan.
The LPA will review two (2) resident files, including medication records, and medication storage.
The LPA will review five (5) staff files, including training. All staff have DOJ criminal record clearance, as required. All staff had required first aid certification and CPR certification.
LPA toured the facility with caregiver Josephine. Hot water was measured at 116. degrees Fahrenheit. All exits were free and clear of obstructions. LPA observed a sufficient supply of food. There was a sufficient supply of hygiene products, paper products, cleaners/disinfectants, linens, furnishings, and personal protective equipment (PPE) supplies for use as needed. Fire extinguishers were serviced and tagged as required. The facility was at a comfortable temperature. The facility had emergency supplies, including food and water as required. All medications were locked up and inaccessible to residents in care. Cleaners/ disinfectants were locked up and inaccessible to residents in care.
