Facility Evaluation Report
Licensing Program Analyst (LPA), Alviso is conducting an Required-1 Year inspection, on 4/15/25 at approximately 3:35pm, and met with Administrator Leonardo Dela Cruz. LPA observed two caregivers, Rolando Correa, and Tsering Dolkar . RN .
Currently there are four (4) clients in care. Facility has an approved fire clearance four (4) non-ambulatory clients. All client rooms are private. The facility does have a required infection control plan. The facility does have a required emergency disaster plan.
LPA toured the facility with the Administrator Leonardo and RN Tsering. Hot water was measured at 118.7 degrees Fahrenheit, which is within regulation. All the exits were observed to be free and clear of obstruction. All resident rooms, hallways, bathrooms, and common areas had sufficient lighting for residents in care. Facility had sufficient food supply. Sufficient cleaners/disinfectants, paper products, hygiene products, and linens. Facility had a sufficient supply of furnishings for client use. The facility was observed to be at a comfortable temperature; Facility does have air conditioning and uses fans as well when needed. The backyard was observed to be clean and orderly, with all pathways clear of obstruction. The fire exit gate opened free and clear as required and needed. All medications were locked and inaccessible to residents in care. All cleaners/disinfectants were locked and inaccessible to residents in care.
The LPA will continue the annual inspection at a later date.
