Facility Evaluation Report
Licensing Program Analyst (LPA) Alviso, conducted a required - 1 year inspection, on 7/21/26 at approximately 11:20am. Licensee contacted Assistant Administrator, Ben Merilatt, from their other licensed home, to meet with the LPA. The House Manager, Hassan Magaye, and DSP, Geraldine Asmat Requena, arrived to the facility after being notified of LPA's arrival.
Facility has a fire clearance approval for four (4) non-ambulatory. Facility has a required emergency disaster plan, and a required Infection control plan. All fire extinguishers, four (4), were observed to be serviced and tagged as required. Hard wired smoke alarms, which include carbon monoxide detection, were working properly during the inspection. LPA did review the emergency drills binder. LPA reviewed requirements of the quarterly drills, with Assistant Manager Ben, including ensuring they are documented, completed quarterly, and held on each shift.
LPA will review four (4) client records, including medication records and medication storage.
LPA will review five (5) staff records, including staff training. All staff had criminal record clearance as required. All staff had first aid certification and CPR certification.
The facility had a sufficient food supply. All exits were free and clear of obstructions. The facility had sufficient lighting in common areas, hallways, bathrooms, and client rooms. There was a sufficient supply of linens, cleaners/disinfectants, paper products, and personal protective equipment (PPE)
