Facility Evaluation Report
Licensing Program Analyst (LPA) Alviso conducted a Required - 1 Year visit, on 10/1/2025 at approximately 10:00am; LPA met with Administrator/Program Director, Reginald Catiis.
Facility fire cleared for thirty (30) non-ambulatory clients. Fire extinguishers were serviced and tagged as required. There are twenty-nine (29) participants enrolled in the day program. There are seventeen (17) staff working today; Six (6) of the enrolled participants have one to one staffing, per needs/care plan.
Facility has a required emergency plan. Facility has a required infection control plan. Facility is conducting emergency drills as required.
LPAs reviewed six (6) client files. All files were complete.
LPAs reviewed seven (7) staff files. All staff have criminal record clearance as required. Staff had current first aid certification, and CPR certification. LPA reviewed annual staff training.,
LPA toured the facility with the Director.Hot water was measured at 107.9 degrees Fahrenheit. All exits were clear and unobstructed. Fire extinguishers, two (2), were serviced and tagged as required. Facility has a carbon monoxide detector as required. Bathrooms were clean and orderly; Bathrooms had soap and paper towels for client use. Facility had a sufficient supply of paper products, cleaners/disinfectants, personal protective equipment (PPE),and furnishings for client use.
