Facility Evaluation Report
Licensing Program Analyst Hansen arrived unannounced to conduct a Case Management inspection and met with Business Office Manager Tolu Faaita as Administrator Brandee Rodriguez was on leave.
LPA is following up regarding an incident that was self-reported by the facility for a medication error. Facility reported a medication error occurring 9/16/2021 for resident, R1 where a medication was not given and another medication of resident was given in it's place. Error was found during an audit. Resident did not have any adverse effects from missing/substituted medication.
The following deficiencies were observed (see LIC 809D) and cited from the California Code of Regulations, Title 22, Division 6 of California Regulation. Failure to correct the deficiency and/or repeat deficiencies within a 12 month period may result in civil penalties. Exit interview conducted and appeal of rights provided. Appeal of Rights Given.
