Facility Evaluation Report

Licensing Program Analyst (LPA) Hansen arrived unannounced for the purpose of conducting a case management inspection and met with Licensee/Administrator Douglas Cattich. LPA observed 3 staff on duty with 8 residents in care.

This case management inspection is to follow up on observations made during annual inspection on 7/22/2025.

LPA toured the facility finding no observed health and safety hazards, home was clean, at a comfortable temperature and well-lit for residents’ safety. The facility was observed to have cable television and phone services, all utilities were observed to be on during the inspection. Staff were observed assisting residents with end of lunch and conducting activities of group singing and storytelling.

During annual inspection facility’s backyard had been under construction for rebuilding of patio. Licensee indicated should be completed by end of summer. During today’s inspection LPA observed completed, cemented portion of backyard (see pics) and per Licensee, prepped, smaller area where in the spring turf will be installed.

LPA also obtained 2 documents that were previously requested & informed by Licensee they will submitting one more document by 10/24/2025.

No deficiencies cited during today's inspection.
