Facility Evaluation Report
At approximately 11:55 AM Licensing Program Analyst (LPA) Stevenson arrived at the facility unannounced to conduct a Required-1 Year inspection. LPA met with Administrator Jennifer Powell. The facility has three clients living at the home. One client was present during my visit, while another was at day program, and a third was away working.
LPA and the administrator toured the facility together to ensure the health and safety of clients. Areas toured include but are not limited to three(3) client rooms, common areas, one (1) bathroom, kitchen, storage areas and back yard. This home is a two level home and the bottom level is not used by clients, and inaccessible with a locked door. In the areas toured no immediate health, safety, or personal rights violations were observed. Common area was clean and in good repair and facility was without odors. All bedrooms had required furniture, bedding, and lighting. Bathroom was clean and in good repair. Facility has a large siting area with bay windows overlooking the Mare Island Bay Area.
Kitchen was clean and in good repair. Food was stored per regulation.
Fire extinguisher was found fully charged and serviced 1/6/2026 and a fire drill was conducted on 01/14/2026. Smoke detectors and carbon monoxide detectors were found operational. Hot water temperature measured within required Title 22 regulations of 105 degrees F and 120 degrees F. All employees requiring background checks are cleared. All required postings are displayed within facility.
