Facility Evaluation Report

01/23/2025 Licensing Program Analyst (LPA) Araceli Canela arrived at the facility unannounced to conduct a Required-1 Year inspection. LPA met with Administrator Jennifer Powell (cert # 6020036735 exp.10-21-24) and explained the purpose of the visit. Administrator stated their Administrator certificate was renewed and waiting for it. The facility has 2 clients living at the home.

LPA Canela and the administrator toured the facility together to ensure the health and safety of clients. Areas toured include but are not limited to two (2) client rooms, common areas, one (1) bathroom, kitchen, storage areas and back yard. This home is a two level home and the bottom level is not used by clients, and inaccessible with locked door. There is a staff bedroom and 2 client bedrooms on the top level. In the areas toured no immediate health, safety, or personal rights violations were observed. Common area was clean and in good repair. All bedrooms had required furniture, bedding, and lighting. Bathroom was clean and in good repair. Kitchen was clean and in good repair. Facility has a locked closet for client medication.

Fire extinguisher fully charged and serviced 1/6/2025 and a fire drill was conducted on 12/29/2024. Smoke detectors and carbon monoxide detector are operational. Hot water temperature measured within required Title 22 regulations of 105 degrees F and 120 degrees F. All employees requiring background checks are cleared. All required postings are displayed within facility. Client files were reviewed and found complete and organized.

No pools/bodies of water are on premises. No firearms are on premises. No deficiencies are being cited as a result of today’s inspection.

Exit interview conducted and copy of report was emailed to Administrator Jennifer Powell.
