Facility Evaluation Report

At approximately 1:30 PM Licensing Program Analyst (LPA) Stevenson arrived at the facility for the purpose of conducting a closure inspection pursuant to voluntary closure of this licensed Residential Care Facility for the Elderly (RCFE) LPA arrived and was allowed in by Heather Muhlestein who has Designation of Facility of Responsibility (RP) and is the sister of Licensee Eric Tanner.

A speaker phone conversation was had with Heather Muhlestein and Eric Tanner, with licensee Eric Tanner confirming that his sister would be signing today's license closure.

During today’s visit, LPA toured the facility, inspected all rooms, the exterior of the building and found no evidence that would suggest that any clients are residing on the premises including any clothing or personal items belonging to clients.

The Licensee initiated this facility closure with plans for voluntary closure and did submit a closure notice on 04/06/2026 to case carrying LPA Shannan Hansen, due to a planned change of use of the facility and need for remodeling. Closure of this facility has been finalized. Facility is closed effective 04/09/2026. RP turned over the License to LPA.
