Facility Evaluation Report

On 12/1/2025 at 8:35 AM Licensing Program Analyst (LPA) Shannan Hansen arrived unannounced to conduct a Case Management inspection of the facility. LPA was greeted by staff Maria who contacted Administrator Eric Tanner by phone. LPA spoke with Administrator who was unavailable but sent Designee Heather Muhlestein. The facility is a Residential Care Facility for the Elderly and has a plan of operations for Dementia care. There are currently 6 residents at the facility, none with dementia diagnosis and or on Hospice.

This facility received Fire Department clearance approval on 2/7/2025 for a total capacity of 6 non-ambulatory residents, with a Hospice Waiver approved for 3 residents. LPA conducted a walk-through of the facility and observed, facility is a single-story house with 4 resident bedrooms, 1 staff room, 2 bathrooms, common areas and garage. Water temperature is within Title 22 regulations of 105 to 120 degrees at faucets used by residents. LPA reviewed files with designee. Facility does not handle resident monies.

Administrator indicated Facilities plan of operations is being updated and will submit to Licensing within the next month. Administrator also confirmed, technical support program appointment scheduled for 12/5/2025 as license date was 3/7/2025.

There were no deficiencies observed or cited during today’s inspection.
