Facility Evaluation Report

03/24/2026, Licensing Program Analyst (LPA) Loera conducted an unannounced Annual Required – 1 yr. inspection visit for this facility. There is currently 1 client in care. Upon arrival LPA was informed client was at day program. Facility is approved for 4 ambulatory clients.

LPA toured the building and grounds. The facility was found to be at a comfortable temperature. LPA observed a 2 day supply of perishable and 7 day supply of non-perishable food. Facility has an emergency food supply.

All rooms were furnished and in good repair per regulation. Extra hygiene products and linens were available. Water temperature in sinks were measured at 112.4 and 114.4 which is within the range of 105 to 120 degrees F. Fire extinguishers were last inspected 05/2025. Smoke/Carbon Monoxide detectors located throughout the facility were tested and operational. Toxins and sharps are located in the staff room and were found to be secured. Client does not take medication. Facility conducts monthly fire drills with the last drill being conduced 03/04/2026.

LPA conducted a review of one client record. All records had the required documentation. LPA conducted review of one staff records/training. Upon a review of staff records, LPA found staff to have current 1st Aid & CPR certification on file.

Administrator certificate for Lori Reese is current from 10/16/2025 - 10/15/2027.

No deficiencies cited during today's inspection.

LPA obtained copies of facilities updated liability insurance and updated lease agreement.
