Facility Evaluation Report
At approximately 9:25 AM, Licensing Program Analyst (LPA) Elias Magdaleno arrived unannounced to conduct a required 1-year annual inspection and was greeted by Nick Paine, Program Manager. Facility is an Adult Day Program (ADP) with thirty-five (35) participants in care. Facility is serviced by vendor North Bay Regional Center (NBRC). Facility operates Monday through Friday 8:00 AM – 2:30 PM.
At approximately 9:35 AM, LPA initiated a tour of the facility with Program Manager and observed the following: Facility is on the bottom story of a multi story building, was a comfortable temperature, and passageways were free from obstructions. Facility's fire extinguishers were observed charged and last serviced 7/25. Smoke detectors and sprinklers are managed by building management and serviced by a third party vendor. Carbon Monoxide detector was tested and operational during inspection. Water temperatures in participant accessible sinks read at 119.6 degrees F which is within the allowable range of 105 to 120 degrees F per Title 22 regulations. Soap & paper towels were available in facility bathrooms. Cabinets containing cleaning supplies and other items that could pose a risk were locked.
Participants bring their own lunches and those who stay at the facility during the day eat in various activity rooms. Refrigerators for participants’ lunches were clean and in good repair. Transportation is arranged partially by facility and LPA observed inspections logs signed off twice daily, first aid kits, and fire extinguishers in a spot check of facility vans. Transportation is also arranged by third part vendors or participant homes. Facility maintains individual schedules for each participant as well as group schedules for staff. Activities include arts, crafting, board games, a computer room, and group outings.
