Facility Evaluation Report

At approximately 9:30AM, Licensing Program Analyst (LPA) Magdaleno arrived at this facility announced to conduct a case management visit in regards to the closure of this facility and met with Administrator Angela Lofton. At approximately 9:35AM, Administrator and LPA conducted a walk through of the building. LPA observed the home to be empty of client belongings and there were no clients present. LPA was informed by Administrator that the last client moved out on Saturday, May 30th. Licensed was forfeited to LPA.

Based on observation, there are no residents present and no one is receiving care or supervision.

Exit interview conducted with Administrator, whose signature on form confirms receipt.
