Facility Evaluation Report
06/18/2026, Licensing Program Analyst (LPA) Loera conducted an unannounced Annual Required – 1 yr. inspection visit for this facility. There are currently three clients in care. All clients were attending day program upon arrival.
LPA and Licensee toured the building and grounds. The facility was found to be at a comfortable temperature. Refrigerated food was found to be stored in a safe manner. All rooms were furnished and in good repair. Extra hygiene products and linens were available. Fire extinguishers were found to be charged and were last inspected April 2025. Smoke/Carbon Monoxide detectors located throughout the facility were tested and operational. Toxins and sharps were found to be locked and secured in a hallway closet. Medications were found to be centrally stored. LPA conducted spot medication count and found start dates and expiration dates to not be filled in. (Technical Advisory)
LPA conducted a review of 3 client records. All records had the required documentation, however LPA observed client (C1) needs and service plan to be updated. Client P&I monies were reviewed and documented. LPA conducted review of 2 staff records/training. Upon a review of staff records, LPA found staff to have required annual and initial training as well as current 1st Aid & CPR certification on file.
Administrator Certificate for Londa Quinn was found to not be current or pending renewal. Administrator will follow up with Administrator Certification Bureau regarding Administrator certificate. (Technical Violation)
