Facility Evaluation Report

05/29/2026, Licensing Program Analyst (LPA) Loera conducted an unannounced Annual Required – 1 yr inspection for this facility and met with Program Manager, Kiana Acosta. The facility currently provides care for 58 clients, 40 of which were at the facility at the time of visit. 21 staff were present during inspection.

LPA conducted a tour of the facility with Program Manager and observed the following; facility was found to be clean and at a comfortable temperature with all exits free from obstruction. Facility has multiple activity rooms, each equipped with sufficient amount of supplies. Fire extinguishers were found to be last charged September 2025. Facility gets fire alarm system inspected and was last tested on September 2024. Facilities last emergency disaster drill was conducted on 05/26/2026. Toxins and cleaning supplies are stored in a designated cleaning supply closet inaccessible to clients. Sharps are stored in a locked cabinet located in the kitchen. LPA was informed clients bring their own lunches to day program.

LPA conducted spot medication count and found prescription medication to be properly recorded on the Centrally Stored Medication Record. LPA conducted a file review of five client records and found all items to be in order. In addition, LPA conducted a file review of five staff records and found staff to have the required amount of annual training and current CPR & 1st Aid.

LPA requested the following documents be sent to CCL by 06/29/2026:

- Updated Certification of Liability Insurance

- LIC610 Emergency Disaster Plan (review, update if needed)

Exit interview conducted and copy of report was provided to Program Manager.
